The Oklahoma Health Equity Campaign (OHEC) will meet to discuss self management of chronic health conditions on Thursday, June 27, from 2 p.m. to 4 p.m. at the Oklahoma State Department of Health (OSDH), 1000 NE 10th St., room 806, Oklahoma City. The meeting will also highlight the OHEC Position Statement on Health and Housing and recent partner developments in promoting affordable and accessible housing.
Speakers from organizations and communities that are implementing the Living Longer, Living Stronger with Chronic Health Conditions self-management program in Oklahoma will discuss the program and answer questions. The six-week community workshops are offered at various locations across the state at no charge and are tailored to reach persons 60 years and older and their caregiver. Persons who are living with various chronic health conditions such as diabetes, arthritis, high blood pressure, heart disease, chronic pain and anxiety, can benefit the most from participating in the community workshops. The workshops provide a valuable tool to help people with chronic health conditions improve their quality of life. Being able to read isn’t a requirement to participate.
At an OHEC meeting earlier this year, Kate Lorig of Stanford University presented the science supporting the health, quality of life and cost benefits of chronic disease self management programs for different populations living in the United States and worldwide. This month’s OHEC meeting will go into more detail about who is implementing the Living Longer, Living Stronger with Chronic Health Conditions program in Oklahoma. Participants will discuss how to get involved in promoting and referring individuals to participate in the six-week workshops in their areas.
